Practice Catchment area
New patients are given an NHS Registration form and a Health & Medical questionnaire to fill in; you will also be offered an appointment for a well person check with the Nursing team. We use this opportunity to collect information on your past medical history, health problems, regular medication and a base line of current health. It is also a good time to check all immunisations, cervical smears and contraceptive requirements are up to date. Unfortunately it can take up to two months to obtain your medical notes from your previous doctor so it is important we collect this information from you directly.
We will be happy to see friends and relatives staying with you if they need to see a Doctor or Nurse.
However, we advise that the relative must contact their own home surgery for advice before contacting Whiteparish Surgery to request an appointment.
